This form is REQUIRED for admission to the University of Tennessee's College of Veterinary Medicine.
- Please fill-in the SUPPLEMENTAL INFORMATION FORM
- Click the SUBMIT button on the form.
- When you have successfully completed the form, you will be directed to our online payment area to pay the required $37.00 fee.
- Animal/veterinary work experience
- Recommendations/evaluations - You are must use the VMCAS electronic application (www.aavmc.org).
You must submit three (3) separate Confidential Evaluation Forms to VMCAS.
