Supplemental Information Form
This form is REQUIRED for admission to the University of Tennessee's College of Veterinary Medicine.

2. Click the SUBMIT button on the form.
3. When you have successfully completed the form, you will be directed to our online payment area to pay the required $37.00 fee.
Animal/veterinary work experience
Recommendations/evaluations - You are must use the VMCAS electronic application (www.aavmc.org).

You must submit three (3) separate Confidential Evaluation Forms to VMCAS.
