Direct Charge Justification
Use this form if you are requesting funds for expenses that are normally not allowed to be charged as direct costs to a project. Examples include, clerical support, computers, phone, fax, etc. This form must be included with the Permission to Apply for External Funding and the Project Budget Form if applicable. Contracts are usually two party agreements and must be signed by individuals authorized to bind the company or university in a legal agreement. The Dean for the College of Veterinary Medicine has been delegated the authority to sign contracts.
Contract Review and Approval Form
The Contract Review and Approval Form is required for all contracts and is created in IRIS using the transaction ZCT_MAINT_CONTRACT (Accounting->Contract Management->Contracts->Maintain Contract). The paper form will no longer be accepted. Complete the form in IRIS (with the exception of the Characteristics section), “save” the form (do not “submit” it), print a copy, and send it, along with the original contract, to Tammy Berry in room A301R for review, negotiation, and signature. The detailed instructions for completion in IRIS can be found at http://rwd.tennessee.edu/nav/index.htm. There, click on Training->Contracts->ZCT_MAINT_CONTRACT – Create Contract (Document).
Sample Research Agreement
Standard Contract - Accounts Payable
Useful for some agreements with no additional terms other than the standard University terms. Typically used for contracting with individuals to provide a service to UT. Download the form.

Useful for a few agreements with small companies that do not have their own contracts for projects that have no intellectual property involvement. Download the form.
Justification of Non-Competitive Purchases and Contracts (Sole Source)
Must accompany contracts that pay over $5,000 to an outside entity without going through the competitive bid process. And must be used for personal contracts over $5,000 per event or $12,000 per calendar year. Download the form.
MTA Questionnaire (outgoing materials)
Material Transfer Agreements are contracts and must be signed by an individual authorized to commit the University to a legal agreement.
