Animal Transfer Request
Transfer of research animals between IACUC protocols must be specifically approved in advance by the IACUC and the Attending Veterinarian or his/her designee. To facilitate the approval process and to maintain appropriate records, an animal transfer request must be submitted to the facility manager who will verify the species, strain, and number of animals against the approved IACUC protocols. The Attending Veterinarian or his/her designee will review the animal transfer form and necessary medical records before granting transfer approval. The signature of the Attending Veterinarian or his/her designee is required prior to initiating an animal transfer. Download Form
Authorization for Veterinarians Providing Clinical Care to Teaching and Research Animals
In cases where responsibility for veterinary medical care will be assigned to a veterinarian other than the University's Attending Veterinarian or his/her designee, the Authorization for Veterinarians Providing Clinical Care to Teaching and Research Animals must be completed and signed by all appropriate parties. Clinical Incident Report
The Clinical Incident Report serves as initial notification of OLAC veterinary staff that an animal has received veterinary medical attention. The form should only be used in cases where OLAC veterinarians are not the veterinarian of record. Click here to electronically complete and/or downloaded the form.
Clinical Case Request
The Clinical Case Request serves as initial notification of OLAC staff that an animal requires veterinary medical attention. The form should only be used to request veterinary medical care in core laboratory animal facilities including CVM-LAF, GSM, WLS, and JH. Completed forms should be given to the facility manager or faxed directly to OLAC at 4-5649. Download Form
Clinical Case Report for Research and Education Centers
The Clinical Case Report serves as initial notification of OLAC veterinary staff that an animal or herd housed at one of the Institute's Research and Education Centers has received veterinary medical attention. For routine conditions, the form should only be completed when disease incidence is above that which is expected. Non-Agricultural Research Animal Acquisition
All acquisition, procurement, or movement of non-agricultural research animals to or from the University of Tennessee animal facilities or laboratories must be specifically approved in advance by the IACUC (via an approved protocol) and the Attending Veterinarian or his/her designee. To facilitate the approval process and to maintain appropriate records, an animal requisition form must be submitted to the facility manager who will verify the species, strain, and number of animals against the approved IACUC protocol. For requests to procure or otherwise acquire non-agricultural animals from non-regulated vendor sources (i.e. animals legally trapped, to be received from a collaborator, donated, acquired from unregulated sources) an animal requisition form must be submitted in advanced and approved by OLAC before the animals are procured. Rodent History Questionnaire and Rodent Quarantine Procedures
Receipt of rodents from non-approved sources must be approved in advance by the Attending Veterinarian or his/her designee. The Rodent History Questionnaire and the Rodent Quarantine Procedures forms, along with the Non Agricultural Animals Requisition forms must be completed to initiate importation of animals for non-approved vendor sources. The facility manager along with the principal investigator should complete appropriate sections of the Rodent History Questionnaire and the Rodent Quarantine Procedures forms. Completed forms should be faxed to 865-974-5649.
